Abstract

ABSTRACT
Health management is the backbone of quality, efficient, and equitable health service delivery. In the era of global
competition and digital transformation, health service facilities are required to adopt modern business
management principles to improve operational efficiency, patient satisfaction, and organizational sustainability.
Metro City, as an educational city in Lampung, has great potential as a center for developing innovation-based
health service management. The purpose of this study is to examine how business management concepts are
applied to health management at healthcare facilities in Metro City and the surrounding areas, pinpoint
implementation obstacles, and develop managerially based methods for maximizing service performance. This
study using structured questionnaires, 180 healthcare professionals and managers from 12 Metro City healthcare
facilities participated in the first quantitative study (Cronbach Alpha = 0.89). The second and FGDs 20 to enhance
quantitative results. The period of data collection was February through June of 2026.Health facilities that
comprehensively applied business management principles showed significant improvements: patient satisfaction
index increased 36.8% (from 61.4 to 84.0 on a 100-point scale), operational cost efficiency increased 28.3%,
health worker productivity increased 39.7%, and patient complaint rates decreased 54.2%. Three dominant
success factors were transformational leadership (OR=4.67), integrated health management information system
implementation (OR=3.89), and competency-based HR development (OR=3.21). The integration of business
management principles in health management has been proven to significantly improve health service
performance in Metro City. Strengthening managerial capacity of health workers through collaboration between
health facilities and universities such as Universitas Wira Buana is key to accelerating health service management
transformation in the region.
