Reproductive health during adolescence encompasses menstrual health and vaginal health, both of which can serve as early indicators of underlying health disturbances. Nutritional status has been hypothesized to affect reproductive function through metabolic, hormonal, immunological, and physiological pathways. This study aimed to describe nutritional status and reproductive health (menstrual complaints and vaginal discharge) and to examine their association among female students of MTs GUPPI 1 Kesumadadi. This quantitative observational study used a cross-sectional design with secondary data from 98 female students aged 13–14 years. Nutritional status was categorized as good, undernutrition, or overnutrition. Univariate analysis used frequency distribution; bivariate analysis used the Chi-square test, complemented by the Freeman–Halton exact test when the assumption of a minimum expected cell count (≥5) was violated. Results showed that 48 students (48.98%) had good nutritional status, 20 (20.41%) had undernutrition, and 30 (30.61%) had overnutrition; 10 students (10.2%) reported menstrual complaints and 5 (5.1%) had abnormal (pathological) vaginal discharge. No significant association was found between nutritional status and menstrual complaints (Freeman–Halton exact p = 1.000). A significant association was found between nutritional status and vaginal discharge status (Freeman–Halton exact p = 0.015): abnormal discharge occurred in 15.0% of students with undernutrition and 6.7% of students with overnutrition, but in none of the students with good nutritional status. These findings suggest that, in this age group, nutritional status may be more strongly related to vaginal/reproductive-tract health than to menstrual cycle disturbance, and highlight the need for classification using BMI-for-age (IMT/U) according to the WHO 2007 growth reference in future prospective studies with individually measured anthropometric and hygiene data.
